Position Summary
Serves as the primary non-clinical liaison between our Home Health, Hospice & Palliative Care Department and the community, supporting patient access, care coordination, and program growth. This role also leads the hospice and palliative care volunteer program, coordinating recruitment, training, and engagement to enhance patient and family support.
This is a full time, non-exempt position
Education & Experience
- High school diploma or GED required
- Associate's or Bachelor's degree preferred
- Experience in volunteer coordination, community outreach, or healthcare preferred
- Minimum of two years of administrative or office experience required
Essential Functions
- Serve as the primary point of contact between HHHPC, patients, families, providers, and community partners, ensuring clear and timely communication
- Build and maintain relationships with hospital departments, clinics, physicians, care facilities, and community organizations to support awareness and referrals
- Educate patients, families, and referral sources on HHHPC services, eligibility, and referral processes
- Represent HHHPC in interdisciplinary meetings, case conferences, and community events, including regular participation in coordination meetings (e.g., Hospital Huddle)
- Coordinate the full volunteer lifecycle, including recruitment, screening, onboarding, training, scheduling, supervision, and retention
- Serve as the primary contact for volunteers, providing guidance, oversight, and support while addressing concerns appropriately
