Job Description:
We are seeking a dedicated Clinical Case Manager to join our team. The ideal candidate will have a strong background in nursing and be passionate about coordinating comprehensive care plans for our patients.
RN Case Manager Key Responsibilities:
- Develop and implement individualized care plans, ensuring patient needs are met and goals are achieved.
- Collaborate with healthcare providers, including physicians and social workers, to ensure comprehensive patient care.
- Provide patient education and support to promote behavioral health and self-care.
- Monitor and report patient outcomes, identifying areas for improvement and developing strategies for improvement.
- Maintain accurate and detailed patient records, adhering to medical records and HIPAA regulations.
RN Case Manager Qualifications:
- Current RN license in New York.
- Experience in homecare preferred.
- Excellent communication and collaboration skills.
- Strong analytical and problem-solving skills.
- Ability to work effectively in a fast-paced environment and prioritize tasks.
- Knowledge of medical records and HIPAA regulations.
