REQUIREMENTS:

• Home Health Aide (HHA) or Personal Care Aide (PCA) Certificate
• Current Photo ID & Social Security Card
• Proof of Address & Direct Deposit Information
• Full Physical (last 6 months) PPD/Quantiferon, Drug Screen, Covid Card Rubella & Rubeola titers
• Two References (1 personal & 1 professional)
