General Description
The Program Manager is responsible for assisting with the oversight, implementation, and monitoring of Day Habilitation services in full compliance with MassHealth Day Habilitation regulations, agency policies, and CARF standards. The position ensures that services are medically necessary, person-centered, properly documented, and delivered as authorized in each participant’s Day Habilitation Service Plan (DHSP). The Program Manager plays a primary role in ensuring audit readiness by maintaining documentation accuracy, staff accountability, participant safety, and service integrity.
Minimum Qualifications
- Bachelor’s degree or three (3) years of directly related experience supporting individuals with I/DD and/or ABI.
- Minimum two (2) year supervisory experience in healthcare or human services.
- Working knowledge of MassHealth Day Habilitation regulations and documentation standards.
- Team Management. Oversight of the daily operations of the Day Habilitation Program
- Experience implementing DHSP goals, writing progress documentation, and participating in IDT meetings.
- Valid driver’s license with safe driving record.
- Ability to pass CORI, fingerprinting, DPPC Registry and background checks.
- Required certifications (or ability to obtain/maintain): CPR, First Aid, PBS, PABC, Human Rights.
Job Responsibilities
Compliance & Audit Readiness
- Ensure services are delivered as authorized by the DHSP.
- Monitor attendance, staffing, and service delivery for compliance and accuracy. Create and distribute staff daily schedule, which includes maintaining ratios.
- Conduct internal file reviews to ensure documentation demonstrates medical necessity and linkage to DHSP goals.
- Support MassHealth audits, corrective action plans, and follow‑up reviews.
- Maintain complete, accurate, and confidential participant records.
Documentation Oversight
- Review Daily Progress Notes, Monthly Ensure daily, weekly, monthly and quarterly data and reports are submitted timely and accurately.
- Progress Notes, and Service Needs Assessments for accuracy and timeliness.
- Assist in managing timelines for all annual support plan material across assigned programs. Assist with reviewing and updating plan material as necessary.
- Compile data to support IDT planning and quality improvement.
DHSP & Interdisciplinary Team (IDT) Responsibilities
- Participate in IDT meetings and contribute to assessments and service planning.
- Ensure staff consistently implement DHSP goals, behavior plans, and support strategies.
- Monitor service delivery during program hours.
Staff Supervision & Training
- Supervise direct support staff including hiring, onboarding, scheduling, evaluations, and corrective action.
- Ensure required staff training, certifications, and competency standards are maintained.
- Provide supervision of daily program activities.
- Flexibility in supporting daily operations as needed, including but not limited to direct care, room or break coverage.
- Coordinate goal implementation and program objectives.
- Provide coaching when documentation or service delivery does not meet MassHealth standards.
- Serve as a certified PABC Instructor and conduct training as needed.
Health, Safety & Participant Well-Being
- Ensure participant’s safety, dignity, rights, and well‑being. Monitor the health and physical/mental well-being of individuals. Respond to concerns immediately and coordinate with appropriate clinical staff to address issues.
- Promote health, independence, and self‑care.
- Monitor incident reporting and corrective actions within required timelines.
- Ensure compliance with Human Rights standards and DSP Code of Ethics.
Quality Assurance & CARF Standards
- Support CARF accreditation and quality improvement initiatives.
- Ensure alignment between CARF standards and MassHealth requirements.
Communication & Representation
- Communicate with MassHealth and external agencies as needed.
- Maintain positive relationships with individuals served, families/guardians, and community partners.
- Ensure quality. Proactive and consistent communication with all guardians, family and team members verbally and in writing.
- Ensure supervisors are aware of performance issues that may negatively affect program operations.
- Assist in the communication of information from management to employees.
- Represent the agency’s mission and values professionally.
Professional Development & Other Duties
- Attend required training courses and remain current on regulatory changes.
- Perform other duties assigned to support program operations and compliance
