Job Description

Vancouver Clinic is hiring a full-time RN Care Manager (36 hours/week) to support our Transitional Care Clinic. In this role, the RN Care Manager will coordinate patient care, helping ensure a seamless, supportive experience for patients and their families. This position is primarily focused on care coordination, patient and family education, psychosocial support, and improving quality of life through compassionate, collaborative care, with additional responsibilities that may include triage, administering treatments, and assisting with minor procedures. The RN Care Manager will embody Vancouver Clinic’s core values of compassion, excellence, integrity, teamwork, safety, and respect while maintaining required competencies for the role. 

 

Schedule: Monday-Friday, 8:00 a.m. to 4:00 p.m.; schedule may vary based on department needs. 

 

Hiring range is generally between $46.10/hour-$58.73/hour and placement in the range is dependent on an evaluation of experience. 

 

Requirements: 

  • Active, unencumbered Washington or Multi-State RN license required. 

  • BSN or MSN/MN degree in nursing (BSN or MSN/MN) strongly preferred 

  • Minimum 2 years clinical nursing experience required with relevant experience in one or more of the following healthcare areas preferred: Coordination of community resources, Care management of diverse patient populations, Ambulatory Care 

  • Case management certification strongly preferred.

Pay Range:

$45.20 - $67.80
