Job Summary

The Dental Health Coordinator (DHC) is a key member of the Department of Pediatric Dentistry, responsible for facilitating patient-centered care coordination across Denver Health's pediatric dental system. Working closely with the Patient Access Specialist (PAS) team, dental providers, and school-based oral health teams, the DHC ensures timely access to care, supports treatment plan completion, and addresses barriers to care for all pediatric dental patients, not just those originating from the School Oral Health Program (SOHP). This role also serves as a lead coordinator for dental navigation efforts across the system, promoting consistency, efficiency, and alignment with departmental and residency program goals.

Essential Functions:

  • Patient Care Coordination:
    • Manage referrals for restorative, urgent, and emergent dental care from SOHP, medical providers, external organizations, and Denver Health dental clinics.
    • Schedule patients directly in ambulatory dental clinics and with School-Based Health Center (SBHC) dentists.
    • Track treatment plans to completion and follow up on missed or incomplete care.
    • Coordinate with perioperative teams and financial counselors, especially for patients requiring general anesthesia or sedation. (20%)
  • Patient & Family Support
    • Assist families with consents, enrollment paperwork, and MyChart setup.
    • Identify and address social determinants of health (e.g., transportation, insurance, language, housing) that impact care completion.
    • Provide education and supportive communication regarding treatment expectations, scheduling, and available resources. (20%)
  • Clinic Workflow & Scheduling Optimization
    • Provide input on clinic scheduling practices to improve workflow, optimize provider utilization, and reduce wait times.
    • Collaborate with PAS team, SOHP providers, and practice managers to refine scheduling guidelines/standards and resolve access issues.
    • Monitor referral queues, appointment backlogs, and patient flow metrics; recommend adjustments as needed. (20%)
  • Departmental Coordination & Leadership
    • Serve as the central coordinator for pediatric dental patient navigation across the Pediatric Dental Residency Program and Denver Health dental clinics.
    • Standardize referral and tracking processes across sites to ensure consistency and efficiency.
    • Facilitate communication between Navigators, PAS team, and department leadership.
    • Support training, resource sharing, and best practice implementation for all dental navigation roles across the system. (20%)
  • Administrative & Program Support
    • Support data collection and reporting for patient access, treatment completion, and grant-related outcomes.
    • Respond promptly to Epic messages, secure chats, and urgent coordination requests.
    • Report progress monthly to the Pediatric Dental Practice Manager and operations team.
    • Perform other duties as assigned to support departmental operations. (20%)


Education:

  • High School Diploma or GED required
  • Associate’s or Bachelor’s degree in health, public health, or related field preferred.
  • Bilingual (English/Spanish) strongly preferred.
  • Completion of Community Health Worker, Patient Navigator, or similar training preferred.


Work Experience:

  • One year of experience in community health, patient access, case management, or dental clinic operations required


Licenses:
Knowledge, Skills and Abilities:

  • Strong organizational skills and ability to manage multiple priorities
  • Knowledge of dental workflows, care coordination, and access strategies
  • Familiarity with scheduling optimization and capacity management
  • Excellent interpersonal communication and motivational interviewing skills
  • Ability to collaborate across teams and coordinate staff in matrixed environments
  • Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint)
  • Experience with Epic scheduling and referral work queues
