Responsibilities
Are you ready to make a difference in the lives of individuals with psychiatric needs? As a Social Worker, you’ll be the go-to person for providing support and services to our amazing patients!
Responsibilities and Accountabilities:
- Knowledge and Understanding: Have knowledge and understanding of Social Work practices and principles.
- Assessment and Planning: Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.
- Patient and Family Communication: Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
- Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
- Team Facilitation: Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
- Clinical Contribution: Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory.
- Effective Communication: Communicate clearly and effectively with clinical team members and Directors to ensure cohesive teamwork.
- Cultural Competence: Be knowledgeable about gender, cultural differences, and age-appropriate care to provide the best support for patients.
Qualifications
- Bachelor's Degree in Social Work Required, with Master’s Degree in Social Work preferred
- Must have licensure in the State of Minnesota
- Experience in healthcare setting preferred
- Experience using electronic health records preferred
- Proficient in Microsoft Office Products including Word and Outlook
- Able to effectively and efficiently communicate in written and verbal form
- Ability to smoothly and rapidly shift between dissimilar tasks
- Ability to communicate well both verbally and in writing
- Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
- Ability to make effective decisions while in stressful situations and circumstances
- After training, be able to manage disruptive and explosive patients.
Compensation for this role ranges from $24-$34 per hour depending on education and experience.
We are committed to providing competitive and equitable compensation. An individual’s pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions. Additionally, we consider internal equity to ensure fairness within our organization.
