Key Responsibilities
Evaluate patients and develop individualized treatment plans.
Provide skilled physical therapy services in accordance with physician orders and established care plans.
Supervise Physical Therapist Assistants and ensure compliance with state practice regulations.
Collaborate with interdisciplinary team members to coordinate and optimize patient care.
Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.
Maintain timely and accurate documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.
Educate patients, families, and caregivers on treatment goals, home exercise programs, and safe mobility techniques.
Assist with discharge planning and recommendations for continued care.
Recommend and facilitate acquisition of durable medical equipment as appropriate.
Monitor and report equipment maintenance needs to ensure a safe treatment environment.
Qualifications
Current Physical Therapist license in good standing.
Strong communication and interpersonal skills.
Commitment to providing exceptional patient-centered care.
