Job Responsibilities:
Assesses and evaluates educational needs of patients and their families. Facilitates patient and family involvement in education and support activities. Coordinates available resources for patients including but not limited to transportation, grocery store gift cards, utility bill assistance letters and foundation applications for patients after review of financial need and patient demographic assessment upon completion of application.
Provides education to help patients make informed decisions, maximize self-care and enhance recovery.
Monitors the environment of care in the Resource room and assures compliance with regulatory agency standards, e.g., infection control, safety, etc.
Circulates throughout the Cancer Center and meets patients during appointment to provide information, education and status updates on resource offerings for patients.
Utilize both DFCI, MGB and SSH respective EPIC platforms along with Quikbase platform for documentation and access to patient electronic medical record.
Communicates identified needs to appropriate members of the patient’s health care team.
Defines goals, develops systems, and work process for the Resource Room. Develops competency in Library World or other library cataloguing program.
Develops policies and procedures for Resource Room and ensures that staff and volunteers are trained and adhere to procedures.
Initiates and directs clinical provider orientation program to the resource room including services offered for patients and families.
Participates and may lead in multi-specialty team meetings with South Shore Health and Dana Farber Cancer Institute along with Cancer Committee Meetings with representatives of medical oncology, radiation oncology, phlebotomy, imaging, and multispecialty/breast care center.
Coordinates Resource room education lectures that focus on the needs of the patient and family population including but not limited to cancer support group offerings and program implementation of additional services including support groups and community events.
Collaborates with respective marketing team to attend community service activities.
Orients staff and visitors to the resource room, including policies and procedures
Creates master list of appropriate books, DVDs, pamphlets and brochures and ensures that sufficient stock is available. Evaluates and updates list periodically.
As necessary, provides reception desk coverage to help navigate and direct patients to respective medical services in the Cancer Center.
Coordinate drop off and pick up of community donation items in partnership with the Foundation and administrative leadership.

Job Requirements
Minimum Education - Preferred:
College degree preferred
Minimum Work Experience:
Some oncology experience preferred
Healthcare experience required
Customer Service experience 2-3 years required
