About the Role:
Join Skilled Nursing Pharmacy as a PM Pharmacist and play a vital role in delivering exceptional pharmaceutical care. We are looking for a dedicated professional to ensure our patients receive the highest quality of medication management and support.
Responsibilities:
- Oversee medication dispensing and ensure accuracy in prescriptions.
- Provide clinical consultations to healthcare staff and patients.
- Monitor patient medication therapy and assess for potential interactions.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Conduct medication reviews and participate in quality assurance initiatives.
- Educate patients and caregivers on medication usage and safety.
- Maintain compliance with state and federal pharmacy regulations.
- Assist in training and mentoring pharmacy staff as needed.
Requirements:
- PharmD or equivalent degree from an accredited pharmacy school.
- Current California pharmacist license in good standing.
- Minimum of 2 years of experience in a pharmacy setting, preferably in skilled nursing.
- Strong clinical knowledge and excellent communication skills.
- Ability to work independently and collaboratively in a team environment.
- Detail-oriented with strong problem-solving abilities.
- Proficiency in pharmacy management software and electronic health records.
