
Discharge Planner, LVN
Job Description
Job Summary
As an integral member of the interdisciplinary team, the LVN Discharge Planner facilitates, develops, and implements discharge planning in collaboration with the Case Manager, Social Worker, patient, family and treatment team. Provides support with the patient, family, and members of the multidisciplinary healthcare team to ensure a safe and effective continuum of care to ensure a smooth transition to the next level of care.
Licensure and Certification:
- Current CA LVN license
- Current BLS provider card
Education:
- Diploma from accredited LVN program
Experience:
- Two years acute care experience
- Prefer one year experience in acute hospital Discharge Planning and Case Management
- Strong interpersonal skills, leadership, negotiation skills, good leadership talent, and knowledge of hospital operations. General knowledge of the payer industry, resource management, reimbursement, and evidence-based clinical practice is essential.
Knowledge and Skills:

- Leadership and negotiating skills
- Maintains positive and proactive communication with patients, families, staff members, and physicians.
- Fosters team-building relationships. Excellent organizational skills and the ability to problem solve are essential.
