Patient Service Coordinator - Non Clinical - Home Health

The Patient Services Coordinator is responsible for scheduling home visits for field staff and coordinating patient care under the direction of the Clinical Manager. This role ensures optimal utilization of clinical resources to support care center capacity and adherence to utilization guidelines, while serving as a liaison between field staff, patients, and the Clinical Manager for scheduling and communication of patient services.
