ABOUT THE JOB

Olivia Hospital & Clinic is looking for a Pharmacist to join our team! This is an exempt position working approximately 64 hours per pay period. The Pharmacist has overall responsibility for medication management, works collaboratively with medical staff and direct care providers, and assures that each ordered medication is appropriate. 

RESPONSIBILITIES

  • Maintains accurate computer formulary; assuring NDC numbers are current and accurate for billing purposes.
  • Work with the Business Office to expedite billing process through regular formulary updates, system reviews, and chart reviews and process improvements.
  • Prepares and dispenses medications including hazardous drugs.
  • Maintains accurate and appropriate patient profiles.
  • Educates patients about their drug therapy when indicated.
  • Assesses patient specific populations and determines appropriateness of drug therapy, including monitoring patients’ response to therapy.
  • Ensures that medications are stored at the appropriate temperatures and using methods such as computerized inventory systems to track expiration dates.
  • Works cooperatively with care providers and medical staff to optimize patients’ drug therapy.
  • Analyzes drug-related problems and develops methods and procedures to resolve.
  • Adheres to ethical standards of pharmacy practice and all rules and regulations that guide the practice of pharmacy.
  • Consults with Physicians and providers to create treatment plans that incorporate appropriate use of medications and other therapies.
  • Researches and provides drug information to medical and patient care staff as needed.
  • Serves on committees, teams and/or workgroups as requested by Pharmacy leader.
  • Participates in Medication Therapy Management.
  • All other duties as assigned.

 

ABOUT YOU

  • PharmD or BS degree from accredited college of Pharmacy (required)
  • Current license with the Minnesota Board of Pharmacy, or eligible to obtain license
  • MTM ACPS certificate or graduation from a college of Pharmacy after May 1996
  • Experience in hospital setting (preferred)
  • Excellent attention to detail and best practice as it relates to safe pharmacy practice (required)
  • Focus attention to details relating to dispensing of medications (required)
  • Effectively deal with occasional high level of stress (required)
  • Knowledge of Electronic Medical Record system (preferred)
  • Knowledge of hood compounding (preferred)
