Job Summary
We are seeking a Home Health Licensed Practical Nurse (LPN) to join our team. In this role, you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You are a compassionate and detail-oriented professional who takes pride in delivering personalized care, observing patient conditions, and reporting changes accurately. Responsibilities
- Provide direct nursing care to patients in accordance with the established plan of car
- Assist in evaluating patient needs and communicate findings to the supervising RN
- Administer medications and treatments as prescribed, and monitor for side effects or adverse reactions
- Document all patient care activities and changes in condition promptly and accurately
- Maintain communication with the interdisciplinary team and follow up on care instructions
- Educate patients and caregivers on basic health and personal care strategies
- Adhere to infection control and safety protocols in the home setting
Qualifications
- Currently licensed Practical Nurse (LPN) in the state of Florida
- Previous experience in home health or similar setting is preferred
- Strong clinical and communication skills
- Valid driver’s license and reliable transportation
- Current CPR/First Aid certification required
