GENERAL PURPOSE OF JOB
The Health Center Lead, Provider is a patient-facing clinician responsible for delivering high-quality care while supporting the day-to-day operations of the health and wellness center. Approximately 90% of this role is dedicated to direct patient care.
In addition to clinical responsibilities, this role provides on-site coordination and operational support to promote efficient workflows, team effectiveness, and a positive patient experience. The Health Center Lead partners closely with Operations Leadership and serves as a resource for staff, helping address routine operational needs and escalate issues as appropriate.
KEY RESPONSIBILITIES
- Provides comprehensive primary care services, including assessment, diagnosis, treatment, and follow-up, in alignment with evidence-based guidelines.
- Performs physical examinations and preventive health services.
- Orders, interprets, and evaluates diagnostic tests.
- Prescribes medications and treatments in accordance with licensure and clinical standards.
- Documents patient encounters, care plans, and outcomes accurately in the medical record.
- Educates patients and families on wellness, prevention, and treatment plans.
- Manages acute and chronic conditions, utilizing available tools and resources to support ongoing care.
- Collaborates with care team members to coordinate services, including referrals to internal and external providers.
- Maintains knowledge of pharmacological treatments, including appropriate use and potential side effects.
- Adheres to all clinical protocols, regulatory requirements, and organizational policies.
- Works collaboratively with clinical and non-clinical staff to support a patient-centered care environment.
- Participates in quality improvement initiatives, patient care programs, and continuing education.
- Supports compliance with clinical, safety, infection control, and HIPAA standards.
- Serves as a clinical resource to team members for day-to-day questions and guidance.
- Supports daily health center operations, helping ensure efficient patient flow and resolution of routine issues.
- Partners with Operations Leadership to maintain effective workflows, staffing coordination, and service delivery.
- Assists with staff scheduling updates and communicates coverage needs or gaps.
- Monitors operational processes such as work queues, logs, and supply levels, escalating concerns as needed.
- Helps address patient concerns and supports service recovery efforts.
- Communicates effectively with staff and leadership regarding day-to-day operations.
- Participates in onboarding, training, and skill development of team members.
- Provides input to Operations Leadership on staff performance and operational improvements.
- Maintains EHR Super User knowledge and supports team utilization of systems.
- Responds to urgent or emergent situations, providing leadership and coordination as needed.
- Completes incident reporting and supports follow-up actions.
- Maintains confidentiality of patient and organizational information.
- Utilizes technology to communicate with patients in accordance with organizational policies.
- Demonstrates professionalism, adaptability, and effective communication in all interactions.
- Performs other duties as assigned.
JOB REQUIREMENTS
Education:
- Completed master's degree level Nurse Practitioner or Physician Assistant program from an accredited college/university.
Experience:
- Minimum three (3) years primary care practice experience.
- 1 year of leadership experience preferred.
- Ability to work independently.
- Working with an Electronic Medical Record (EMR).
- Experience working with pediatric patients in some locations.
- Certain clients and health centers may require travel.
- Certain clients and health centers may require phlebotomy.
Certificates, Licenses, Registrations:
- Active and unrestricted applicable licensure.
- Current CPR (BLS) certification or must be obtained within 60 days of hire.
- NPI & DEA.
- Current National Board Certification.
- May be required to obtain and maintain a valid DOT Medical Examiner Certification, depending on job responsibilities or location.
