Key Responsibilities – Behavioral Health RN (Home Health)
- Perform in-home behavioral health and psychiatric nursing assessments, including mental status and safety evaluations
- Deliver skilled home health nursing interventions, including medication management and symptom monitoring
- Provide patient and caregiver education on mental health conditions, treatment adherence, and coping strategies
- Develop and implement individualized behavioral health care plans in a home and community care setting
- Coordinate care with physicians, therapists, and interdisciplinary teams
- Maintain accurate, timely documentation in accordance with home health standards
Qualifications – Behavioral Health Nurse (RN)
- Active California Registered Nurse (RN) license in good standing
- RN-BC certification is required
- Minimum 1 year of behavioral health or psychiatric nursing experience (home health or community care preferred)
- Graduate of an accredited nursing program (BSN preferred)
- Strong skills in clinical assessment, crisis intervention, and therapeutic communication
- Public Health Nurse (PHN) certification (preferred)
- Valid driver’s license and ability to travel throughout Ventura County
