Patient Care Navigator

Works as a liaison and collaborates with physicians, orthopedic providers, patient/family, nursing, utilization review, and other healthcare team members to assure patient management that efficiently and effectively aligns with patient needs using resources to meet quality, clinical, and cost-effective outcomes. This role supports our innovative Digital Clinic and Care Management Programs and blends orthopedic care with virtual tools to streamline patient engagement, improve outcomes, and enhance the orthopedic care experience.
