Care Navigator (Outbound)

Upperline Health is seeking Care Navigators with prior healthcare experience, to support a team of clinicians in delivering complex plans of care in accordance with the patient’s health status and overall goals and values. This individual will be responsible for providing all telephonic and back-office support required by the team of clinicians.
