Sign-On Bonus Eligible
This position is eligible for a sign-on bonus up to $5,000 for external candidates. Payout is based on employment in the exact role at the time of payout. Internal team members are not eligible.

- Travel required throughout Near North City and North Suburbs
How You Will Make a Difference
In this role, you will support patient care delivery as part of the home health team, working under the direction of the Clinical Manager. You will:
- Assist with patient care activities for a designated group of patients
- Support the delivery of care aligned with Advocate Health at Home standards
- Help ensure patients receive high-quality, cost-effective care
- Promote and support patient and family rights
- Contribute to maintaining and improving overall quality outcomes
Required Qualifications
- Active license in the State of Illinois
- Minimum of 2 years of medical-surgical (med/surg) experience
- Comfortable using computer-based scheduling and documentation systems
- Strong written and verbal communication skills
- Effective interpersonal and teamwork skills
Compensation & Benefits
Hourly Pay Range: $24.35 – $36.55
Actual compensation is based on experience, skills, and internal equity.
