Job Description:
Program Expertise & Clinical Services
This role involves serving as a key contact for the immunization program, providing technical guidance on vaccine administration, supporting community health initiatives, and assessing community needs to develop effective health and disease prevention plans.
It also includes assisting providers with program compliance, including eligibility, vaccine storage, and documentation, offering expert knowledge on vaccine policies and procedures, and utilizing data management software to ensure smooth program operations.
Compliance Monitoring & Site Visits
The Community Health Educator will be conducting site visits to evaluate community nursing and immunization program compliance, reviewing records, performing audits, and developing corrective action plans to ensure quality healthcare services.
It includes data management, reporting, analyzing trends, and collaborating on educational resources. The role also entails reviewing staff performance, implementing quality improvement initiatives, and participating in policy development.
Communication and Outreach
Additionally, it involves community outreach, counseling, liaising with healthcare partners, supporting the NESIIS help desk, and performing any other duties as assigned to promote immunization and public health.
Requirements / Qualifications
Minimum Qualifications: Post high school coursework/training in community health education, biological science, adult/continuing education or related field AND experience in public/community health education activities with responsibility for: developing and evaluating health education programs; determining health needs; preparing educational and informational materials, training aids, and presentations; or conferring with individuals and groups to exchange information pertinent to community health education programs.
Preferred Qualifications: Education: bachelor's degree in health education, public health, health promotion or a related field and a valid driver's license.
Other: Valid driver’s license or the ability to provide independent authorized transportation
