The Medical Social Worker responsibilities include, but are not limited to, the following:
- Conduct psychosocial assessments (emotional, cultural, financial needs)
- Assist with evaluation and referral for suspected abuse/neglect
- Implement social service interventions; counsel patients/families on illness, treatment, and grief/conflict
- Assist with advance directives and durable power of attorney for health care
- Collaborate with Case Managers and care teams on discharge planning
- Act as liaison between patients, families, care teams, and external agencies; facilitate care planning
- Maintain required documentation in accordance with policies
- Report to assigned manager or director
- May require additional physical demands or respiratory fit testing
- Follow standard precautions due to potential exposure to infections, blood/body fluids, and chemicals

REQUIRED QUALIFICATIONS
EDUCATION:
Master's in social work (MSW) from an accredited school
Hospice and Home Health:
This job requires the incumbents to operate an INTEGRIS-owned vehicle OR personal vehicle (non-INTEGRIS-owned) and therefore must have a current Oklahoma State Driver's License as well as a driving record which is acceptable to our insurance carrier.
