Clinical Pharmacist

Join Mosaic's integrated care team as a Clinical Pharmacist — a pivotal role at the intersection of patient care, provider collaboration, & medication safety. Working across all Mosaic clinics, you'll be embedded in multidisciplinary teams alongside providers & nursing staff to ensure every patient receives medication therapy that is safe, effective, & cost-conscious.

In this role, you will assess, implement, & monitor medication-related therapies — catching therapeutic incompatibilities, supporting adherence, & engaging patients through face-to-face, telephonic, and electronic care models. You'll serve as a trusted drug information resource for care teams, deliver education to patients and staff, & help shape the future of patient-centered pharmacy practice within our growing system.
