REGISTERED NURSE CASE MANAGER - Field Work Required
Job Description
The hospice nurse Case Manager helps coordinate and administer hospice care for patients, families and caregivers through the utilization of hospice resources. The hospice nurse is experienced in nursing with an emphasis on community health education and experience. The hospice nurse utilizes hospice and community resources to help coordinate and drive services through a plan of care to meet the needs of patients and families. RN $38.00 - $47.50 per hour RN $35.00 - $47.50 per hour
Shifts Available:
Weekend Hospice RN -on-call; Saturday and Sunday 8p-8a on-call; with competitive hourly wages plus differentials Berkshire County area
Evening Hospice RN -On-call (2pm-10pm) Monday-Friday with competitive hourly wages plus differentials Berkshire County area
