Position Overview
The Waiver Service Coordinator (WSC) is responsible for conducting in-home assessments, identifying support systems, and coordinating long-term services under the MyCare Ohio Waiver Program. This role ensures members receive appropriate care in home and community-based settings while maintaining compliance with Medicaid and program regulations.
What You’ll Do
- Conduct comprehensive in-home assessments to evaluate biopsychosocial needs, functional impairments, and support systems
- Identify and address health and safety risks
- Develop and implement Person-Centered Service Plans in collaboration with members and caregivers
- Monitor and adjust service plans based on changing needs
- Participate in Interdisciplinary Care Team (ICT) meetings
- Coordinate waiver services in alignment with payer sequencing requirements
- Communicate with providers to resolve service delivery issues
- Maintain ongoing contact with caregivers and support systems
- Advocate for members and caregivers
- Monitor Medicaid financial eligibility and waiver compliance
- Conduct annual reassessments (or more frequently as needed)
- Support audits and compliance reviews
- Assist with disenrollment processes and appeal education
- Maintain accurate, timely documentation
- Ensure HIPAA and confidentiality compliance
- Promote strong customer service and positive employee relations
Qualifications
- Licensed Social Worker (LSW) or Registered Nurse (RN) with an active, unencumbered Ohio license
- Minimum 1 year experience in home care, disability services, or case management
- Experience conducting health/social interviews
- Knowledge of human behavior, family systems, and community resources
Requirements
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to pass annual Motor Vehicle Record (MVR) check
- Ability to work using dual-monitor systems and multiple databases
- Maintain LSW or RN licensure (required for continued employment)
- Obtain and maintain Ohio Department of Aging certification
- Adhere to HIPAA and AOoA confidentiality standards
Work Environment
This position is primarily office-based in a climate-controlled environment with regular in-home visits. Requires extended computer use and occasional lifting to 25 lbs.
