Compensation - $32-38/hour
Boost Home Healthcare is a Medicare-certified, ACHC-accredited skilled home health agency serving the Greater Boston area. We are building a high-quality, patient-centered team and are looking for a motivated LPN to join us as a Clinical Coordinator in a dual-role capacity — both in the field and in the office. You will be playing a central role in referral intake, care coordination, and office and business operations with the opportunity to provide patient care services in the field as needed.

What you’ll be doing:

Office (Clinical coordination)
- Assist with administrative tasks in office. Assist with patient intake, insurance verification, and onboarding
- Support care coordination: scheduling visits, tracking authorizations, and communicating with referral sources
- Participate in case conferencing and team huddles
- Help maintain OASIS timeliness and clinical documentation compliance
- Support Case Managers in securing the accurate clinical information required for service patient care
- Receive and scribe orders into EMR system as needed

Field (Patient Care)
- Perform skilled nursing visits in patients’ homes per RN Plan of Care
- Provide wound care, medication management, IV therapy, and other LPN-scope interventions
- Document visits accurately and timely in the EMR
- Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Communicate patient status changes to the supervising RN or DON
What we’re looking for:
Excellent communication skills, written and verbal English skills required. Spanish or another language a bonus.
- Strong data entry and general computer skills required.
- Graduate of an approved school of practical (vocational) nursing.
- Licensed practical (vocational) nurse in the state of job posting.
- Minimum of one (1) year as an LVN in the Home Health Care Setting.
- Must have current BLS Certification.
