Job Posting: Nurse Practitioner (NP) – Transitional Care
(Full-Time, 3-Month Contract) April – June 2026 | 4 Positions Available

An organization in partnership with Nova Scotia Health is seeking four (4) full-time Nurse Practitioners (NPs) to provide interim coverage from April through June 2026, supporting a Transitional Care Facility prior to permanent NP hires. This is a unique opportunity to practice in an innovative, restorative model of care, within a collaborative inpatient environment

About the Transitional Care Facility
-The facility supports adult patients (18+) in Nova Scotia’s Central Zone who: No longer require acute care (medically ALC with non-medical discharge barriers)
-Require additional time, services, and support before safely returning to the community
-Benefit from a restorative, activating environment focused on regaining and maintaining functional independence

Role Overview: Nurse Practitioner 
-Provide full-scope post-acute inpatient care for 22–25 patients
-Be onsite Monday–Friday
-Establish Expected Date of Discharge (EDD) within 24–48 hours of admission
-Communicate discharge planning with families and the care team
-Diagnose and manage acute and chronic conditions
-Support smooth transitions to home/community through collaboration with the NSH Outreach Team
-Complete documentation in relevant EMR
-Collaborate with pharmacy partners on medication management

Ideal Candidate
-We are seeking NPs who:Are licensed and in good standing to practice in Nova Scotia
-Have experience in inpatient, post-acute, geriatric, or transitional care settings
-Thrive in collaborative, interdisciplinary team environments
-Are passionate about restorative care and supporting patients to return home safely
-Are available full-time for the full April–June contract period
