PACT-Y Outreach/ Case Manager

This is an exciting opportunity to be a part of Metro-Boston’s PACT-Y team through the Home for Little Wanderers.  PACT (Program for Assertive Community Treatment) is an evidence-based home/community centered model.

The PACT-Y Outreach/Case Manager provides intensive, community-based support to youth with significant emotional and behavioral health needs as part of a multidisciplinary team.
