The RN Case Manager is responsible for identifying and monitoring patients with chronic medical conditions or those included in assigned clinical metrics, determined quarterly by the Chief of Medicine. This work involves conducting comprehensive and ongoing patient assessments through review of lab results, medication adherence, observation, physical examination, and patient interviews. The position determines case closure by evaluating patient needs, collaborating with OSH staff, and coordinating with receiving entities. Responsibilities include obtaining medication authorizations, supporting medication management, and acting as a liaison between pharmacy and physicians. The role facilitates referrals, provides follow-up services, and promotes quality, cost‑effective patient care while adhering to professional case management standards. It requires independent decision‑making, engagement with high‑risk patient populations, and serving as a central referral contact across departments. Additional duties include participating in interdisciplinary meetings, coordinating admissions, transfers, and discharges, and assisting social work with establishing community providers to ensure continuity of care for complex patients.

Possess a valid Oregon Professional Registered Nurse License at the time of appointment.
Special Qualifications: These qualifications are required in addition to meeting the minimum qualifications.
A Bachelor of Science in Nursing plus 2 years of nursing experience providing medically focused patient care, OR
An Associate Degree in Nursing plus 5 years of nursing experience; 2 of those years providing medically focused patient care.

Skilled in coordinating care, determining case closure, collaborating with multidisciplinary teams, and following case management standards and protocols.
Ability to review lab results, evaluate medication adherence, conduct physical exams, observe patient behaviors, and perform patient interviews.
Effective in working with physicians, pharmacy staff, social work, and external providers to ensure continuity of care.
Ability to make independent judgments with limited direction, analyze information, and determine the best course of action.
Comfortable participating in team meetings (IDT, PET) and working across departments to support admissions, transfers, and discharges.
Good understanding of medication authorization processes, collaboration with pharmacy, and use of treatment algorithms as guided by providers.

This announcement is for one, full-time, permanent, AFSCME represented RN Case Manager (Mental Health Registered Nurse) based in Salem, Oregon.

Employment is contingent upon a criminal records check that meets OHA criteria at the time of hire and throughout employment.
The salary listed is the non-PERS qualifying range. If the successful candidate qualifies for PERS, the salary range will increase by 6.95%. For more information, see the Classification and Compensation page.
