Job Summary

The Social Worker NE applies principles of Social Work and behavioral health practices to provide patients and families with resources, support, and the best individualized plan of care for each family member. This includes member/family assessment, intervention, support, advocacy, and case management. Remains an active member of a multidisciplinary teams which include PCPs, nurses, psychologists, psychiatrists, other health plan social workers, child protective service staff and/or public health nurses to support the member and family. Is responsible for identifying the needs of the member and providing appropriate resources such as parent support groups, transportation, housing, financial, recreational, and library services. Actively maintains a current resource library accessible to all CHA staff and pursues and maintains close relationships with community providers. Assists families with obtaining behavioral therapy (ABA) services. Consistently monitors the status of these services and works closely with the Regional (RCOC) to assist with transition of care when appropriate.

Pay Range

Minimum $37.61 Midpoint $49.84 Maximum $62.06

Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.

Experience

* Required: Pediatric hospital experience or field placement at a children's hospital.

* Preferred: One (1) year of experience with children and families in a healthcare setting.

Education

* Required: Master's degree in Social Work from an accredited school of Social Work.

* Preferred: LCSW highly preferred.

Training

* Preferred: Training or experience in Foster Care, Mental Health, Domestic Violence, Child Abuse, Elder Abuse.

Specialized Skills

* Required: Leadership, communication, conflict resolution, cultural sensitivity. Ability to work effectively under pressure due to changing priorities. Excellent interpersonal communication, including conflict resolution, problem solving, and negotiation skills. Clinical knowledge of multiple age groups, medical illness, and treatment plans according to disease process and diagnosis. Strong organizational and time management skills, as evidenced by capacity to multi-task and prioritize. Ability to work independently and exercise sound judgment in interactions with physicians, patients, and their families. Demonstrates the knowledge and skills necessary to communicate with internal and outside resources and agencies. Must be proficient in required competencies upon completion of orientation and maintain annual departmental competency requirements. Ability to work collaboratively regardless of social, economic and cultural backgrounds. Knowledge of computer systems and software used in functional area.

* Preferred: Bilingual in English/ Spanish.

Licensure

* Required: Basic Life Support (BLS) - required within 90 days of hire.

* Preferred: Licensed Clinical Social Worker
