Community Health Worker

This is a Hybrid position (3 days on-site and 2 days remote). This role may require occasional adjustments to on-site requirements based on operational needs. The Community Health Worker (CHW) supports the Quality Improvement department by engaging patients, coordinating outreach activities, and assisting with closure of clinical quality gaps. The CHW builds trusting relationships with patients and connects them to care, education, and resources that support improved health outcomes. This position functions exclusively within the Quality Improvement department and collaborates closely with clinical leadership, informatics, and operations to support quality initiatives, data integrity, population health priorities, and equitable care delivery.
