- Full time, 40-hour position
- Requires availability for 4, 8, & 12 hour day and night shifts
- Normal Day shift - 7 a.m. to 7 p.m.
- Normal Night shift - 7 p.m. - 7 a.m.
- The Lead may be required to work a mix of Day and Night shifts. The Lead schedule may incorporate overlapping shifts with another teammate to provide ample patient coverage
- All Inpatient Intervention Specialists may be required to work a rotation of weekend shifts
- The Intervention Specialist Lead may be scheduled for shift gaps or call offs to ensure ongoing patient coverage.
Pay Range:
$22.90 - $34.35The Lead supports leadership by providing onsite guidance and support to the team responsible for inpatient care for individuals who have experienced recent trauma. This role assists with on-call schedules, fosters collaboration with internal and external stakeholders, coordinates hospital and trauma clinic schedules, ensures data quality, monitors productivity, and fulfills other key responsibilities under the direction of the leadership team.
Major Responsibilities:
Team Navigation: Assists the leadership with daily operations and team performance by guiding the team to meet individual and team goals, high standards of productivity, quality documentation, interdisciplinary collaboration, and fostering a supportive work environment.
Patient Support: Provides immediate emotional support and resources to hospitalized patients following a trauma, including connecting them with the clinical team when appropriate, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. Also, complete assessments and facilitate warm-handoffs to providers for ongoing care.
Resources Navigation: Responds to trauma alerts, safety calls, pre-escalation needs and collaborates with street outreach teams and facilitates mediation to mitigate retaliation risks.
Outreach: Guides the team, offers patient support, works alongside other groups, and actively engages in intensive community outreach. This also involves making follow-up telephone calls and sending letters to discharged patients to connect them with outpatient resources.
Continuity of Care: Schedules patient appointments with the TRC, other internal Advocate Health resources, or community partners before discharge and provides brief follow-up calls after discharge when necessary, to ensure ongoing care which requires strong collaboration with other providers.
Assessments: Conducts intakes and needs assessments, linking patients to appropriate resources and outpatient services. It may also require notifying other hospital providers of unmet needs to be addressed while in the hospital.
Record Keeping: Maintains accurate records of patient interactions and action plans.
Collaboration: Works with the hospital treatment team and community partners to ensure comprehensive treatment, discharge planning, and continuity of care.
Minimum Job Requirements
Education
High School Diploma or GED.

Experience
Minimum of three years' experience in social services.
Knowledge / Skills / Abilities
- Strong verbal and written communication skills.

- Proficiency in Microsoft Office Suite.
- Strong organizational skills.
- Ability to handle difficult conversations and maintain professionalism.

Physical Requirements and Working Conditions
- Full-time position (40 hours per week), with flexibility to cover all shifts as needed.
- Availability to work days, evenings, nights, weekends, and holidays.
