Position Summary

The Care Coordinator supports patient care by identifying health maintenance needs, coordinating services, and assisting with administrative tasks to improve outcomes and reduce gaps in care. This is a telephonic role and does not involve in-person patient care.

Scheduling & Logistics:
This is a hybrid position. During training, employees are required to work onsite Monday through Friday. After successful completion of orientation and demonstration of independence, the schedule transitions to two days onsite and three days remote per week. Candidates must reside within a reasonable driving distance of the WRO office, as the role will remain hybrid and will not become fully remote. Standard hours are Monday through Friday, 8:00 AM to 4:30 PM, with limited flexibility in start and end times; core business hours must be maintained. No weekends or holidays are required.

Job Summary:

Responsible for detailed preview of charts for high complexity patients and patients in the population health programs within the virtual care center. Works with providers across the organization to ensure accurate coding for diagnosis. Assists in providing cost-effective healthcare services through telephonic outreach. Serves as a liaison between patients, physicians and all other members of the interdisciplinary health care team.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Reviews charts to identify ongoing health maintenance needs and facilitates next steps to assist patients in meeting those needs. 
  • Obtains outside medical records and assists with uploading into the electronic medical record. 
  • Schedules patient appointments and assists licensed personnel to enhance comprehensive patient care and reduce gaps in care. 
  • Works in constant state of alertness and safe manner. 
  • Performs other duties as assigned. 

EDUCATION

  • Bachelor’s degree in a healthcare related field or equivalent combination of education and experience

PHYSICAL REQUIREMENTS

  • Constant standing and walking.
  • Frequent use of hearing and speech to share information through oral communication.
  • Frequent use of hearing to distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent reaching, gripping, and keyboard use/data entry.
  • Frequent use of vision for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Occasional bending, stooping, crawling, kneeling, sitting, squatting and repetitive foot/leg and hand/arm movements.
  • Occasional use of vision to judge distances and spatial relationships.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Occasional lifting/moving patients.
  • Occasional use of smell to detect/recognize odors.
  • Occasional driving.
