Full-Time | $50,000–$53,000 per year | On-Call for Incident Reporting

At ViaQuest Healthcare Central, we are committed to providing high-quality, person-centered care to individuals with developmental disabilities in our Intermediate Care Facilities (ICF/IID). We are currently seeking a dedicated and compassionate QIDP to join our team.

Key Responsibilities
- Ensure compliance with all state and federal ICF/IID regulations
- Develop, implement, and monitor Individual Plans (IPs)
- Conduct annual, quarterly, and special team meetings
- Complete Comprehensive Functional Assessments and required documentation
- Maintain accurate and up-to-date floor books and records
- Monitor implementation of services and supports in day programming
- Coordinate appointments and community-based services based on individual needs and preferences
- Oversee medication and medical supply management, as appropriate
- Participate in incident reporting (on-call rotation)
Qualifications
- Bachelor’s degree in Social Services or a related field
- Minimum 1 year of experience working with individuals with developmental disabilities
- Strong communication, organization, and customer service skills
- Ability to manage multiple priorities and meet deadlines
- Supervisory or leadership experience preferred
