Social Services Director (Full-Time)
All Saints Subacute & Skilled Nursing is seeking a dedicated and compassionate Social Services Director to join our team. This position plays a vital role in enhancing the quality of life for our residents by providing social services support, advocacy, counseling, and discharge planning in a skilled nursing environment.

Position Summary
The Social Services Director is responsible for providing medically related social services to residents to help attain or maintain the highest practicable physical, mental, and psychosocial well-being. The ideal candidate possesses strong organizational, communication, planning, and interpersonal skills while maintaining objectivity, confidentiality, and professionalism in all resident interactions.
Essential Duties and Responsibilities
Establish courses of action by exploring options and setting goals with residents and their families.
Assist residents in achieving the highest practicable level of self-care, independence, and overall well-being.
Provide medically related social services to support residents' physical, mental, and psychosocial health.
Evaluate social, family, psychological, and emotional factors to assess social service needs and develop individualized care plans.
Provide emotional support and counseling to residents and families coping with illness, disability, grief, loss, and health-related concerns.
Assist residents and families with healthcare decisions and care planning.
Assist residents with personal shopping needs as appropriate.
Maintain inventory and tracking of resident belongings.
Coordinate responses and investigations regarding missing, lost, or stolen personal belongings.
Connect residents with community resources, social service agencies, health organizations, and welfare programs.
Assist residents in obtaining prosthetics, adaptive equipment, and assistive devices.
Develop and maintain collaborative working relationships with facility departments and community agencies.
Document resident social service assessments, interventions, and progress.
Assist in scheduling outpatient appointments and coordinating ancillary services, including optometry, podiatry, dentistry, and psychiatric services.
Participate in discharge planning and coordinate referrals, equipment, agency services, and follow-up care with the interdisciplinary team.
Provide in-service education and training to staff as requested.
Counsel residents and family members regarding psychosocial concerns and adjustment issues.
Qualifications
Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.
Experience in skilled nursing, subacute care, long-term care, or healthcare social services preferred.
Knowledge of discharge planning, resident rights, community resources, and regulatory requirements.
Strong verbal and written communication skills.
Excellent organizational, documentation, and problem-solving abilities.
Ability to maintain confidentiality and establish positive relationships with residents, families, staff, and community partners.
