Job Description
The WOCN provides expert care and coordination for patients requiring wound, ostomy, and continence management in the home setting. This role supports high-quality patient outcomes through advanced clinical practice, staff education, and collaboration with the interdisciplinary care team.
This position will cover Western Nassau and Queens.
- 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
