Job Summary: The Nurse Navigator – CHF serves as a clinical guide and advocate for patients with heart failure, coordinating care across the continuum to support optimal outcomes. This role provides patient and family education, facilitates communication between multidisciplinary care teams, and ensures adherence to evidence-based guidelines. The Nurse Navigator proactively supports quality improvement initiatives and contributes to a consistent, patient centered experience within the HVI Service Line.
