SCOPE OF ROLE: ACT is an intensive, community-based multidisciplinary treatment team that provides person-centered care to a shared caseload of adults whose needs are not able to be met through traditional outpatient services. In collaboration with a psychiatric care provider, the team nurses assist with medication management and administration, through documentation as well as monitoring clients for medical problems or side effects.
