Essential Functions:
Meet and exceed revenue productivity.
Collaborate with clients to complete Intake/Discharge packets and other administrative paperwork.
Elgin Residential: 3 hours of billable case management per 8-hour shift. Elgin Recovery Home: 3 hours of billable case management per 8-hour shift. Kenmore Residential: 3 hours of billable case management per 8-hour shift. Kenmore Recovery Home: 3 hours of billable case management per 8-hour shift.
Adhere to all compliance rules regarding treatment services and contracts.
Follow and adhere to all timelines regarding documentation completion.
Understand and reflect the clinical model of the program.
Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
Position Qualifications:
High school diploma or equivalent required.
Associates, bachelors, or master's degree in a social service field preferred.
Background check clearance required.
Annual TB test required.
Demonstrated proficiency in EHR documentation and review required.
Experience helping people create change preferred.
Clinically trained and experience using best practices as well as experience working with a variety of client ages, issues, and symptoms preferred.
Familiarity with accreditation and licensure standards preferred.
Experience establishing and maintaining collaborative community partnerships preferred
Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
Valid driver’s license, in good standing for the state of residency required.
Access to reliable transportation required.
Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
