Spec, Healthcare Coordinator III - Englewood

Pay: $25.71 to 27.00 DOE Health Care Specialist III, works with the resident to develop appropriate case plans and goals and provide on-going assessment and support. Assists the residents to utilize community resources to meet individual needs. Works with residents to identify their skills and abilities to promote personal growth. Develop strategies to link residents with health care and care coordination services. This position is not eligible for a hybrid work schedule.
