Job Description
Highland Hospital is seeking a Full Time RN Case Manager to join its growing team. This newly created role will help support Highland Hospital's Red Creek Cardiology office. Candidates for this role will work Monday - Friday.
Under general direction of the Lead APP, the RN Care Manager’s central role integrates and coordinates access and utilization management, proactive patient management, care facilitation and treatment planning functions. The RN Care Manager manages clinical aspects of patient centered care, working with the interdisciplinary medical team in the provision of patient care. May participate in supervision of other clinical staff members.
Salary Range:
$37.13- $45.00 an hour
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
