Program Support Assistant

TRANSITIONAL SUPPORT SERVICES (TSS) provide intensive case management and wrap around community support services to adults (18+) with co-occurring mental health and intellectual or developmental disabilities. All services support the successful integration and stability in communities. Individuals gain a sense of purpose, independence, and knowledge to lead a meaningful life.
