GENERAL OVERVIEW:
Responsible for leading professional development for Care Management services across assigned areas of the health system. This role designs and evaluates onboarding, annual education, continuing education, and competency validation to support safe, compliant, and effective Care Management practice. The position partners with leaders and interdisciplinary teams to advance transitions of care, care coordination, and practice excellence. Travel to multiple hospital sites and care settings is required to support education, onboarding, and competency activities.
ESSENTIAL RESPONSIBILITIES
Collaborates with Care Management leadership and interdisciplinary teams to support orientation, ongoing education, quality improvement initiatives, and practice excellence related to transitions of care and care coordination. (50%)
Plans, develops, implements, and evaluates Care Management-focused educational and professional development programs, utilizing appropriate methods, measurements, and tools to assess achievement of learning and competency outcomes. (20%)
Serves as an educational consultant to leadership and staff in areas of Care Management practice, regulatory and accreditation requirements, professional development, program development, and assigned committee work. (15%)
Coordinates, implements, and maintains Care Management onboarding programs, including role-specific orientation, annual education requirements, and continuing education initiatives. (10%)
Conducts formal and informal learning needs assessments and develops, implements, and oversees the annual Care Management competency and education program, including competency validation and documentation in collaboration with leadership. (5%)

QUALIFICATIONS:
Required
Master’s degree in Social Work, Nursing, Education or relevant experience and/or education as determined by the company in lieu of Master's degree.
2 years of experience in Hospital-based Care Management clinical
2 years of experience in Education and training
Current State of PA RN or PA SW licensure or current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC)
Certification in Case Management (CCM®, ACM®, or equivalent) within 3 years of hire.
CPR certification
Act 34 Criminal Background Clearance Certificate
Act 33 Child Abuse Clearance Certificate
Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
