Job Description

This role will support one of our new clinics opening this fall (anticipated October 1, 2026). The expected start date for this position will fall between August 25 and September 8, 2026, allowing time for onboarding and hands‑on training at our Hudson and Spring Hill clinics prior to opening. Additional details about the training timeline and expectations will be reviewed during the interview process. 

About the Role

As a Referral Coordinator, you keep patients moving through their care journey. You own the referral process end‑to‑end—making sure every referral is accurate, authorized, scheduled, and communicated clearly. You’re the connector between patients, providers, and specialists, ensuring seniors get the care they need without delays or confusion.

This is detail‑driven, people‑first work where your organization, communication, and follow‑through directly shape patient outcomes. You’ll report to the Center Manager and support the clinical team, with opportunities to take on more responsibility as we grow.

Location: Brooksville clinic (some local travel may be required to help cover other centers).

Anticipated Clinic Opening: October 1, 2026
We are hiring ahead of go‑live so our new teams are fully trained and ready to deliver exceptional care on day one.

Training & Onboarding: 
Training will occur at our Hudson and Spring Hill clinics prior to opening.

Office Hours: Monday–Friday, 8am–5pm

What You’ll Do

Own the referral, start to finish

  • Submit referrals and authorizations with complete, accurate information
  • Track approvals, denials, and next steps to keep care moving
  • Coordinate records and documentation so specialists have what they need
  • Document thoroughly in the EMR and maintain clean, accurate patient files

Keep patients informed and on track

  • Schedule specialist appointments and follow up until completion
  • Verify insurance and eligibility to prevent delays
  • Respond to patient and provider inquiries with concierge‑level support
  • Provide clear instructions so patients know exactly what to expect

Be the connector across teams

  • Partner with providers to create seamless patient experiences
  • Collaborate with clinical staff to keep care coordinated
  • Support front office and MA workflows when the clinic needs an extra hand
  • Contribute to care team discussions that strengthen patient outcomes

Improve the work over time

  • Identify workflow gaps and help refine referral processes
  • Maintain a patient‑first mindset while upholding clinic standards
  • Embrace a team-first mindset by supporting clinical and front office operations while growing your skills along the way

About You

You bring:

  • 3+ years as a referral coordinator or medical assistant, ideally in primary care
  • 2+ years managing referrals from primary care to specialists
  • Strong communication skills that build trust with patients, providers, and teammates
  • The ability to multitask, prioritize, and stay organized in a fast‑paced clinic
  • Compassion, professionalism, and a patient‑first mindset
  • High attention to detail and commitment to excellence
  • Composure and focus when things get busy
  • A collaborative spirit and willingness to learn

  • Are a Certified Medical Assistant (CMA)
  • Have experience supporting both front office and clinical workflows
  • Are bilingual in English and Spanish
