Job Description

The Case Manager will;   Responsibilities: - Conduct intake process with new family members within 24 hours of admission.  This includes but is not limited to explanation of client’s rights and responsibilities and grievance procedures. - Conduct psychosocial, vocational/educational, and housing assessments and documentation of same. - Meet weekly to develop and monitor individual and family service plans in conjunction with family members to meet the family’s needs. - Provide child care referrals and assist families to navigate the NYC Department of Education system. - Establish linkages and...
