Position Summary
The Central Clinical Services Coordinator provides administrative and care coordination support to the centralized nursing and clinical services team. This role is responsible for facilitating transitions of care workflows, managing medical record retrieval and organization, supporting provider inbox activities, and coordinating patient scheduling to ensure timely follow-up and continuity of care.
Job Description
PRIMARY DUTIES AND RESPONSIBILITIES
- Retrieve, organize, and upload medical records from hospitals, specialists, and other care settings.
- Support Transitions of Care workflows, including preparation of patient cases for nursing outreach.
- Monitor and manage assigned provider inboxes and clinical work queues.
- Schedule post-discharge follow-up appointments and other clinically indicated visits.
- Route documents and information to appropriate clinical team members.
- Track outstanding record requests and follow up as needed.
- Document activities accurately within the electronic health record (EHR).
- Collaborate with nurses, pharmacists, providers, and care team members to support patient care and quality initiatives.

POSITION QUALIFICATIONS
Education:
- High School Diploma or equivalent.
- Associates Degree in healthcare administration, business or other related field.
Experience:
- 1+ years of healthcare, medical office, scheduling, or administrative experience.
- Experience in primary care, care management, transitions of care, or population health.
Knowledge:
- Knowledge of medical terminology and healthcare workflows.
Skills:
- Proficiency with EHR systems and Microsoft Office/Excel
Abilities:
- Strong communication, organizational, and customer service skills.
