Physical Therapist (PT) – Home Health

Territory: Mt Vernon, Kingstown, Arlington, Alexandria City

Schedule: Mon-Fri 8 AM-5 PM

Position Summary

As a Home Health Physical Therapist (PT), you'll provide skilled physical therapy services under physician orders to help patients regain mobility, restore function, reduce pain, and maximize independence in the comfort of their homes. The PT develops individualized plans of care, educates patients and caregivers, ensures patient safety, and collaborates with an interdisciplinary team to deliver exceptional outcomes. The PT is responsible for accurate clinical documentation and maintaining compliance with Medicare, state, and agency regulations.

Qualifications

Required

  • Current Physical Therapist (PT) license in the state of employment.
  • Minimum of one (1) year of PT experience in an acute care setting.
  • Valid driver's license, reliable transportation, and current auto insurance.

Preferred

  • Home Health PT experience.
  • Experience with OASIS documentation and Medicare regulations.

Key Responsibilities

Patient Care

  • Provide skilled PT services in patients' homes according to physician orders and the established plan of care.
  • Perform comprehensive patient evaluations, including OASIS assessments, and develop individualized treatment plans.
  • Complete initial evaluations and admissions within established timeframes.
  • Deliver therapeutic interventions to relieve pain, improve mobility, restore function, and maximize independence.
  • Reassess patients regularly and adjust treatment plans based on progress and changing clinical needs.
  • Implement strategies to reduce hospital readmissions through early intervention and patient education.

Patient & Family Education

  • Educate patients and caregivers on disease management, therapeutic exercises, fall prevention, and home safety.
  • Instruct patients and families on the safe use of assistive devices, including walkers, canes, crutches, wheelchairs, braces, and prosthetic or orthotic equipment.
  • Encourage active participation in the plan of care and promote patient independence.
  • Collaborate with the appropriate team members regarding durable medical equipment (DME) needs and financial resources.

Care Coordination

  • Collaborate with physicians, nurses, PTAs, and other interdisciplinary team members to ensure coordinated, high-quality care.
  • Supervise PTAs and complete required supervisory visits.
  • Promote continuity of care through appropriate admissions, transfers, and discharges.
  • Communicate changes in patient status and scheduling needs with the care team.
  • Participate in case conferences and interdisciplinary care planning.

Documentation & Compliance

  • Complete accurate, timely clinical documentation in accordance with agency policies, Medicare guidelines, and federal and state regulations.
  • Document evaluations, treatments, patient progress, assessments, and education.
  • Complete chart audits and quality assurance activities as requested.
  • Maintain an active PT license and comply with all professional practice standards.

Professional Responsibilities

  • Participate in on-call rotation as assigned.
  • Attend mandatory meetings, in-service trainings, and continuing education.
  • Assist with orientation and training of new team members when requested.
  • Maintain a professional appearance and represent the organization with integrity.
  • Follow all agency policies, procedures, and regulatory requirements.

Essential Functions

  • Travel to patients' homes throughout the assigned service area.
  • Maintain reliable attendance and punctuality.
  • Complete visit documentation within required company timeframes, including Start of Care, evaluations, recertifications, resumption of care, routine visits, and discharge visits.
  • Deliver care within the scope of practice for a licensed PT.

  • Drive between patient visits.
  • Lift up to 50 pounds with or without assistance.
  • Stand, walk, bend, reach, stretch, and stoop.
  • Use computers, tablets, and electronic medical records.
  • Read, write, and communicate effectively in English.
  • Hear and communicate effectively in person and by phone.

Occasionally Required

  • Distinguish odors and tastes as necessary for patient care.
