Health and Wellness Specialist - OP Care Management - TEMPORARY EMPLOYEE DAYS

POSITION SUMMARY Supports and advocates for patients and their families pursuit of health and wellbeing. Helps provide navigation through various health and social services to obtain resources that support the adoption of a healthy lifestyle. Works closely with the primary care team, the Wellness Center, and resource providers to promote and improve the health of our patients. Provides health coaching, education, and information counseling with a focus on self-management and health promotion. Extensive community outreach, such as home visiting, is required.
