Community Health Worker

The Virginia Department of Health (VDH) is seeking a motivated and community-focused Community Health Worker to support public health initiatives within the Rappahannock-Rapidan Health District. The Community Health Worker serves as a liaison between community members and health and social service systems, helping individuals access resources, navigate services, and improve health outcomes. This position supports outreach, health education, community engagement, and public health programs designed to promote the health and well-being of individuals, families, and communities across the district.
