Job Description
A Day in the Life
- Working in partnership with other case managers, physicians and patients, you will identify and assist patients with complex post-acute HIV needs and facilitate the development of a safe discharge plans.
- You will be responsible for assessing the patient’s continuum of care needs, developing a plan in collaboration with the patient the healthcare team, and implementing the plan using the resources of the Resource Center.
- You will advocate for the patient and family throughout the entire process.
What’s Required
Current Michigan licensure as RN, LLBSW, or LBSW required, or eligible for licensure within a timeframe determined by Human Resources.
Experience working with persons living with HIV and/or in medical case management preferred but not required.
Certification as an HIV Case Manager and HIV Testing/Counseling through the State of Michigan to be completed within the first year of employment.
Possesses a valid driver’s license and is eligible for Class III Chauffeur license.
Demonstrated oral and written communication skills and competency in Microsoft Office products and electronic medical record systems.
