ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Collaborates with Senior Director of Healthcare Navigation, Vice President of Patient Navigation and applicable leadership in The System, maintenance and continued development of patient transitions to The System and community outreach programs.
  • Conducts nursing assessments and observations of patients referred for admission and similar nursing visits for current hospitalized Health System patients.
  • Conducts high level presentations and facilitates meetings with key referral source representatives.
  • Represents The System in the field in order to facilitate referrals and admissions, streamlines the referral process, and conducts referral visits and patient assessments.
  • Assists with obtaining referral documentation and physician orders when necessary.
  • Communicates with applicable system locations regarding patient transitions.
  • Collaborates with applicable locations within The System in the establishment and maintenance of transitional activities.
  • Coordinates new and existing relationship building with major and minor referral sources.
  • Assists The System in constructing strategic initiatives designed to explain and educate providers and the community about what The System provides.
  • Establishes and maintains professional relationships with all referral sources, including but not limited to case managers, discharge planners, social workers, physicians and physician practices, and any other community or third party referral sources. 
  • Maintains thorough knowledge of market/community in which PTD services in regards to potential new referral sources, hospital discharge trends, and any other relevant demographic information relevant to the continued growth of The System.
  • Maintains awareness of competitors in the market, and develops profiles.
  • Maintains working knowledge and technical skill for the use of hospital referral portal platforms.
  • Responsible for meeting established goals for referrals, admissions, conversion ratios, and other quality metrics determined by leadership.
  • Reviews monthly analytic data.  Tracks information to determine market trends, and develops action plans based on data relative to key performance indicators.
  • Attends and participates in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
  • Attends and participate in mandatory in-services and trainings
  • Honors patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
  • Complies with Corporate Compliance Program.
  • Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
  • Follows established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
  • Follows established safety procedures when performing job tasks and/or working with equipment.
  • Performs other related duties as necessary and as directed by supervisor.
  • Complies with all Privacy & Security programs.
  • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
  • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
  • Performs other duties as necessary to ensure the success of the System.

SKILLS AND ABILITIES

  • Excellent customer service, interpersonal and oral/written communication skills.
  • Knowledge of long-term care admission requirements.
  • Knowledge and understanding of Medicare, Medicaid, Private Pay and Third Party reimbursement.
  • Ability to multi-task and function with little direction.
  • Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing.
  • Communicates well with patients and family members providing warm and friendly greeting and an approachable attitude to family/visitors/patients and responds to expressed concerns while displaying a helpful, caring demeanor and answers questions when appropriate in a professional manner.

MINIMUM QUALIFICATIONS

  •  Associates or Bachelor’s degree in Nursing and one year related Experience in one of the following:  marketing healthcare products or services, long-term care, inpatient rehabilitation or health care community relations.
  •  RN required
  • Valid Georgia Driver’s License.

SUPERVISORY RESPONSIBILITIES

Provides supervisory oversight for Patient Transitions Directors.  Will hold Patient Transition Directors accountable for performance relative to established goals and key performance indicators as well as professional development.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram format.  The ability to deal with problems involving several concrete variables in standardized situations.
